With the NHS reshaping general practice, the financial roadmap built around partnership may no longer exist for many GPs. What that means for doctors at the start of their career – and why they should start to adapt now.

It used to be that the GP career ladder was straightforward – if not easy.

For the past forty years or so, doctors going into general practice trained, went on to work as associates, progressed towards partnership, built equity in a practice, and established financial security in the process. The destination was clear, and most GPs who wanted partnership could reasonably expect to reach it. That’s all starting to change.

Neighbourhood-based integrated care is already rolling out. The first wave of neighbourhood health services launched in September 2024. Some GPs are already navigating employment structures that would have been unthinkable five years ago. If you've recently qualified, you might be entering one of these organisations right now.
