A newborn baby died as a result of catastrophic injuries caused by prolonged use of instruments and likely excessive traction during an attempted forceps delivery, an inquest has found.

In November 2024, Ada Rose died aged one month old after acquiring severe spinal and skull injuries during her birth at Queen’s Hospital, Romford.
