A hospital was found to have committed serious failures in communicating a terminal diagnosis to a patient.

Doctors at the Countess of Chester Hospital did not tell father-of-seven William Chapman about his deadly pulmonary fibrosis diagnosis, a Parliamentary and Health Service Ombudsman (PHSO) investigation determined.
