NHS electronic patient records linked to ‘persistent’ safety risks, watchdog warns

Electronic patient record systems used in the NHS are associated with “persistent” patient safety risks despite a raft of reports and national recommendations, a watchdog has found.

The Health Services Safety Investigations Body (HSSIB) reviewed 63 patient safety incidents between 2018 and 2025 and found issues with both the design of electronic patient record (EPR) systems and how new systems had been implemented.
