A coroner has issued a stark warning after the "preventable" death of a young man from sepsis, exposing how non-clinical staff made critical decisions without sufficient skills to do so safely.

Adam Ali Hussain died aged 23 on 16 May last year at Queens Medical Centre, Nottingham, from complicated appendicitis with perforation leading to peritonitis, abdominal sepsis and multiple organ failure.
