Patient death after five-day scan delay exposes dangerous confusion between hospital teams

A coroner has identified critical systematic failures at a hospital after an 88-year-old woman died following a five-day delay in obtaining a CT scan to diagnose an incarcerated hernia.

A prevention of future deaths report has been issued to Tameside and Glossop Integrated Care NHS Foundation Trust following the death of Winifred Wardle on 19 February 2025 from respiratory failure and pneumonia following emergency hernia repair surgery.
