Patients ‘coming to harm’ due to inadequate discharge summaries, report finds

Patients are “coming to harm” as challenges in local health and care services have not been accounted for in their discharge planning, an investigation has found.

The Health Services Safety Investigations Body (HSSIB) published a report into electronic communications during patient discharge from hospitals.
